2. If you want to have a Written Contract, one will be provided and will have to be signed before the patient drive can be set up.

Before You call to Verify it with Carolyn- you will not be placed on the Event Calendar! SO BE SURE TO MAKE AN APPOINTMENT WITH CAROLYN TO ENSURE YOUR PATIENT DRIVE WILL BE SET UP AND THE CONTRACT WILL BE ACTIVE!
